SMBP Membership Application.
- Firstly, please make sure you have made an account using your company email.
Have your vet records ready.
In this form we will also require your dog(s) vet records, they are uploaded to this form as a document form.
If you need to upload more documents or the records as an image from your camera roll/desktop, you can do that via the customer service chat window on the bottom right
Applications submitted without records will not be considered.
What Vaccines are required?
Vaccine requirements are:
DPP (Distemper, Parvovirus & Parainfluenza)
Bordatella
Rabies
+ spayed/neutered if 8 months and older
Influenza & Lepto Shots are strongly encouraged but not a core requirement. We also recommend getting your Bordatella shots every 6 months if your dog(s) frequent dog parks.
Application Process
For SMBP applications We first confirm the application is valid & a current tenant of SMBP. Once processed and approved, you will be invited to visit the park for a temperament assessment before purchasing a membership.
The assessment is so that you can check out the park, and we can check out your pup(s).
This temperament test will be the final step in your application process.
